Long before science had a name for it, people talked about “gut feelings” and “butterflies in the stomach” everyday language for something researchers now understand quite literally.

The gut and the brain are physically wired to each other through a dense, two-way communication network known as the gut-brain axis, and information travels along it constantly, in both directions.

This matters more than it might seem: there are more nerve cells in the gut than almost anywhere else in the body outside the brain itself , and disruption on one side of this axis is increasingly recognized as capable of surfacing on the other, in digestion, mood, sleep, and clarity of thought alike.

With World Brain Day observed every July 22 by the World Federation of Neurology (WFN), and this year’s global theme centered on “Brain Health:    Access for All”, it’s a fitting time to look closely at one of the most studied everyday influences on brain health: what happens in the gut.

Why the Gut and Brain Are Connected

The connection starts with the enteric nervous system (ENS), a network of more than 500 million neurons lining the gastrointestinal tract, complex enough that scientists sometimes call it the “second brain”.

The ENS can sense conditions inside the gut and respond locally, but it also stays in close contact with the brain through the vagus nerve, the primary channel carrying sensory signals from the gut upward and motor signals from the brain back down.

Layered on top of this nerve pathway are the endocrine and immune systems: hormones that signal hunger, fullness, and stress, and immune activity that shapes inflammation levels throughout the body, both of which influence, and are influenced by, brain function.
None of these systems work alone; together they form a continuous feedback loop between digestion and the brain.

Gut Microbiota: The Hidden Contributor

Within the gut lives a vast ecosystem of bacteria, fungi, and other microorganisms collectively known as the gut microbiota. These microbes do more than aid digestion, they produce or help produce many of the neurotransmitters that carry messages between gut and brain, and they generate other compounds that can reach the brain through the bloodstream.

Serotonin is a notable example: research estimates that roughly 90% of the body’s serotonin, a neurotransmitter closely tied to mood regulation, is produced in the gut rather than the brain itself. Gut bacteria also produce short-chain fatty acids, which help regulate immune activity and inflammation, a pathway increasingly linked in research reviews to mood and stress reactivity.

Because this relationship runs in both directions, the brain and gut can also reshape each other’s environment which is part of why chronic stress and digestive symptoms so often appear together.

What Can Disrupt the Gut–Brain Axis?

The gut–brain axis is designed to keep the digestive system and the brain in constant communication. However, modern lifestyle habits can gradually interfere with this delicate balance. When these signals become disrupted, communication between the gut and the brain becomes less efficient, affecting multiple systems throughout the body.

Several factors are known to contribute to gut–brain axis dysfunction, including:

  • A diet high in ultra-processed foods and low in dietary fibre, which reduces microbial diversity.
  • Chronic psychological stress, which alters gut motility, increases stress hormones, and changes the composition of the gut microbiota.
  • Poor or insufficient sleep, which disrupts both the gut microbiome and the body’s circadian rhythm.
  • Physical inactivity, which is associated with reduced microbial diversity and metabolic health.
  • Excessive alcohol consumption and smoking, both of which can impair the intestinal barrier and promote inflammation.
  • Frequent or unnecessary use of antibiotics, which may disturb the balance of beneficial gut bacteria.
  • Gastrointestinal infections or chronic inflammatory conditions, which can damage the gut barrier and alter communication with the brain.

 

These factors may contribute to gut dysbiosis—an imbalance in the gut microbiota—and increased intestinal permeability. As inflammatory molecules and microbial by-products enter the bloodstream more easily, they can influence immune activity, hormone regulation, and neural signaling, ultimately affecting brain function.

How Gut–Brain Axis Dysfunction Can Affect Your Health

The gut–brain axis is designed to maintain constant communication between the digestive system and the brain, allowing both organs to work together to regulate digestion, metabolism, immune responses, mood, cognition, and overall health. Under healthy conditions, signals travel continuously through the nervous system, hormones, immune pathways, and the gut microbiota to help the body adapt to changing internal and external environments.

However, this communication network can become disrupted. Chronic psychological stress, poor dietary habits, inadequate sleep, certain medications such as antibiotics, infections, and long-term inflammation may all alter the composition of the gut microbiome and interfere with signaling between the gut and the brain. Researchers increasingly refer to this imbalance as gut–brain axis dysfunction.

Rather than causing a single disease, dysfunction of the gut–brain axis appears to influence multiple biological systems simultaneously. As a result, researchers have linked it to a growing number of digestive, neurological, metabolic, and mental health conditions.

1. Disorders of Gut–Brain Interaction (DGBIs)

The digestive tract is usually the first place where disruption of the gut–brain axis becomes noticeable.
Modern medicine now classifies many chronic digestive conditions as Disorders of Gut–Brain Interaction (DGBIs), recognizing that these conditions involve abnormal communication between the gastrointestinal tract and the nervous system rather than structural abnormalities alone.

The most common examples include:

  • Irritable bowel syndrome (IBS)
  • Functional dyspepsia
  • Functional constipation
  • Functional diarrhea
  • Functional abdominal bloating
  • Functional abdominal pain

Unlike inflammatory bowel diseases such as Crohn’s disease or ulcerative colitis, many DGBIs occur despite normal endoscopic findings or laboratory investigations. Instead, symptoms appear to result from altered gut motility, increased visceral sensitivity, immune activation, intestinal permeability, and changes in the gut microbiome.

Stress often plays a significant role. Activation of the hypothalamic–pituitary–adrenal (HPA) axis increases cortisol production, which may alter intestinal permeability (“leaky gut”), influence gut bacteria, and modify communication through the vagus nerve. This helps explain why many people experience worsening digestive symptoms during periods of emotional stress.

Research also suggests that alterations in microbial diversity may reduce the production of beneficial metabolites such as short-chain fatty acids, further affecting immune regulation and intestinal barrier integrity. For many patients, digestive symptoms and psychological symptoms coexist, highlighting the bidirectional nature of the gut–brain axis.

2. Mental Health and Mood Disorders

The gut–brain axis also plays an important role in emotional wellbeing.

Although mental health disorders are influenced by genetic, environmental, and psychosocial factors, increasing evidence suggests that the gut microbiome may contribute to mood regulation through several biological mechanisms.

Gut microorganisms influence the production and metabolism of numerous neuroactive compounds, including serotonin precursors, gamma-aminobutyric acid (GABA), dopamine precursors, and tryptophan metabolites. They also interact with the immune system, helping regulate inflammatory pathways that have been associated with depression and anxiety.

Chronic gut inflammation may increase circulating inflammatory cytokines, which can affect brain function either directly or indirectly through immune signaling pathways.

Meanwhile, prolonged stress may alter gut microbial composition, creating a cycle in which stress affects the gut while gut dysfunction may further influence stress resilience.
Researchers have reported associations between gut microbiome alterations and conditions including:

  • Anxiety disorders
  • Depression
  • Chronic stress
  • Fatigue
  • Reduced stress resilience

 

Importantly, these findings do not suggest that depression or anxiety originate solely in the gut. Instead, the gut microbiome is increasingly recognized as one of many biological factors that may influence mental wellbeing.

3. Neurological Function and Cognitive Health

Communication between the gut and brain extends beyond mood to cognitive performance and neurological health.

Emerging evidence suggests that changes in the gut microbiome may influence attention, learning, memory, and executive function through immune signaling, microbial metabolites, endocrine pathways, and the vagus nerve.
